The hospital's Health Information Management (HIM) team is able to assist you with obtaining copies of your medical records Monday - Friday from 8:00AM to 5:00PM.
To obtain copies of medical records, you will be required to submit documentation sufficient enough to establish your identity and/or right to access confidential patient health information. This could include, but is not limited to, a valid photo ID, passport, birth certificate, death certificate, marriage license and other verifiable documentation.
All requestors for confidential patient health information must sign a release of information before records will be released unless a court order or subpoena is presented.
Also, medical records can be obtained online by signing up for our patient portal.
