FMLA: Family and Medical Leave Act

McCurtain Memorial Hospital offers FMLA leave to full-time employees who have worked for the company at least 12 consecutive months and worked at least 1250 hours over the last 12 months. If you believe that you are eligible for FMLA and meet the guidelines set forth by the United States Department of Labor, as well as MMH policy, there are resources below to help you in this process. 

Step 1: Understanding your Rights and Responsibilities under the FMLA. We've attached a poster for you to review which will explain your rights and your responsibilities as an employee who is requesting leave under the FMLA. 

Step 2: Tell us what type of leave under the FMLA you are requesting. There are different types of leave that you can request pursuant to the FMLA. You will want to notify us of the type of leave you are requesting so that we can provide you with the necessary documents to complete and return to us. The types of leave you may request are here:

Step 3: Make certain all documents are completed and returned timely. We will be emailing you several types of documents during the FMLA review process and thereafter. It is extremely important that you return these to us timely. You can upload and send us documents here:
