Family and Medical Leave Act

McCurtain Memorial Hospital, pursuant to state and federal law, affords eligible employees leave under the Family and Medical Leave Act ("FMLA"). This page is intended to provide basic information to you and resources, as well as afford you a link by which you may inform us of your desire to apply for FMLA leave.

As a general rule, to qualify, you must have worked with us at least 12 months and/or worked a minimum of 1250 hours during that preceding 12-month look-back period.

FMLA is a federally regulated leave. Please review the Rights and Responsibilities poster to the right for important information about FMLA, please. If you have any questions at all, you should direct them in writing to fmla@mmhok.com or simply complete the form below and submit it. We will respond to you within 12 hours on the first day of business following your submission.

How to Apply? - Read Carefully

Click the below "APPLY HERE" link. You will be redirected to Kronos to login to your personal account. From there, you will click "MY TIME" and under that tab, you will see an option to Request Leave for FMLA.
