Welcome to McCurtain Memorial Hospital! We're super excited that you have chosen to join our team and wanted to take this opportunity to share some of our benefit information with you. Below, you will find our 2023-2024 Benefits Guide and Enrollment Form. You will want to review this package and complete the form. We've made the forms printer friendly so feel free to print them off, complete them, sign and return to Human Resource - or, come by and our friendly HR staff will assist you. Please do not forget: you only have 15 days from your date of employment (not start date) to complete and return your benefit enrollment forms to Human Resource.

Employee's only have 15 days from the date of their hire, not your start date, to complete your insurance selections. Please review the 2023-2024 Benefits Guide and the Life Insurance Guide. The two enrollment forms are above and can be completed by clicking the image. If you need assistance, come to Human Resource for assistance.
